Agent Context

The OPSD loop shipped WRITE-ONLY (audit project_learning_loop_audit_2026_07_02): insights bank but never feed forward. bin/session-insights (#342) already prints the SessionStart additionalContext JSON but isn't wired as a live hook. Wire it via bin/install-agent-docs into the ~/.claude + ~/.codex SessionStart hook config. Beware feedback_settings_json_midsession_silences_hooks: editing settings.json mid-session can silence hooks -> verify live injection in a FRESH session. North star: a new agent hatches with +100 insights.
