Agent Context

The 'Narrate your trajectory' section (canonical docs/agents/index.md, generates root AGENTS.md) currently gets ZERO adoption — a fresh session without an explicit prompt does not narrate at all. Make it NATURAL CONTEXT: promote it from a soft buried subsection to a PROMINENT, high-priority instruction — near the top of the operating model, framed as a REQUIRED part of how every agent works (like the DevOps gate), so agents narrate WITHOUT being asked. Keep it tight but LOUD/unmissable. Include the bin/atomic-event start/end usage (and the new boundary call 'next --outcome ... --category ... --reason ...' from the parallel event-outcomes task). Regenerate the root adapters (bin/install-agent-docs) and verify. Docs-only, no app code. This is the operator's #1 goal: narration should be automatic, not prompted.
