Agent Context

OPSD self-learning MVP task-zero. Forward-only AtomicAction capture: one record per agent action (seq, type, input, output, outcome, per-action model/tokens/cost, feedback_anchor). Foundation for heartbeat view, feedback grading, insight bank. No backfill. Design in scratchpad atomic-heartbeat.html + greenfield-atomic-trajectory.md; memory project_opsd_learning_layer.
